    Morphological Study of the Cyclic Changes in the Endocervical Epithelium

    In order to examine the cyclic changes in the endocervical epithelium, fresh human endocervical specimens were obtained from 60 cases for observation by light microscopy (LM), scanning electron microscopy (SEM), and transmission electron microscopy (TEM). The results obtained were as follows: 1) The endocervical epithelium was composed of ciliated cells and secretory cells at a ratio of 1:5-10 which did not vary by menstrual cycle, pregnancy and age. 2) Cyclic changes of ciliated cells were not observed in the menstrual cycle, but decrease in number and tip-thinning of the cilia were found in the postmenopausal phase by SEM observation. 3) Dilatation and complexity of the glandular lumen and increase in cell size reached a peak in the ovulatory phase. The findings of secretory cells in the first trimester of pregnancy were the same as those of the ovulatory phase and atrophic changes and simplified formation of the gland were observed in the postmenopausal phase. 4) SEM observation showed surface projections of various sizes and slightly dilated, round and shortened microvilli on the secretory cells in the ovulatory phase and the first trimester of pregnancy. During the postmenopausal phase, atrophic changes such as flattening of the cell surface and decrease and shortening of microvilli were observed. 5) TEM observation showed a peak in secretory granules and organelles in both number and size and exocytotic secretion during the ovulatory phase and the findings observed during the first trimester of pregnancy were similar to those during the ovulatory phase.A reports based on this study was presented at the 34th Annual Meeting of the Chugoku & Shikoku Districts of the Japan Society of Obstetrics and Gynecology held in Y onago on October 10, 1981

    After reading Professor Ali Farazmand's essay "Building Administrative Capacity in the Age of Rapid Globalization: A Modest Prescription for the Twenty-First Century," one is reminded that the list of capacities we need to develop is long, the tasks are diverse, and the challenges are tremendous. By offering a "modest" prescription, Farazmand departs emphatically from recent attempts to rewrite administrative capacity by downgrading it and outsourcing it to the market or by making it imitate corporate structures and techniques. Such exercises of yesteryear are correctly being excoriated now, but it would be well to remember that this failed model was inspired and facilitated by the stagnation and many perversities of the state-centered model that it replaced. As we stand at the cusp of a new generation of public administration reforms, we are reminded by Professor Farazmand's exposition that we no longer have any widely accepted and integrative theories left to guide our practice of reform, and that a search must begin to look at past evidence with a critical eye toward developing practical and theoretical insights to advance the public administration agenda

    Surgical Resection for Local and Lateral Lymph Node Recurrence of MSI-high Cecal Cancer with the BRAF V600E Mutation

    An 84-year-old female underwent open right hemicolectomy with D3 lymph node dissection for cecal cancer, pathologically identified as pT4aN2M0 Stage IIIc and BRAF mutation-positive. Due to early recurrence of abdominal wall and right lateral lymph nodes, the patient was treated with FOLFOXIRI+Bevacizumab. Imaging after 5 courses of chemotherapy found tumor shrinkage and no new metastases. The patient did not tolerate chemotherapy well, and tumor resection was performed. Microsatellite instability (MSI) testing using multiplex polymerase chain reaction (PCR) fragment analysis revealed MSI-high status. The patient is currently recurrence-free without chemotherapy at 1 year postoperatively. BRAF-mutated colorectal cancer has a poor prognosis, and may require resection of the metastatic or recurrent tumor after comprehensive evaluation

    Two Cases of Dizygotic Twins with Androgenetic Mole and Normal Conceptus

    Two cases showing a molar placenta with a fetus/infant were analyzed for karyotype and enzyme polymorphisms and found to be dizygotic twin pregnancies with an androgenetic mole and a normal conceptus. One case resulted in spontaneous abortion at 19 weeks' gestation and was subsequently diagnosed as an invasive mole and required chemotherapy. The other case was a conception following clomiphene plus human menopausal gonadotropin (HMG) therapy and resulted in a normal female infant born at 33 weeks. It is emphasized that, since dizygotic twins in which one is an androgenetic mole have a high propensity for malignancy, they should be carefully differentiated from partial moles with which they might be confused by analysis of genetic polymorphisms and they should be followed up with care

    Descending Colon Cancer Coincident with Schistosoma japonicum in an 89-year-old Male

    An 89-year-old male came to the hospital with a complaint of abdominal distension. Abdominal computed tomography showed wall thickening in the descending colon and marked dilatation of the proximal colon, and lower gastrointestinal endoscopy demonstrated a stenosis in the descending colon. Although a biopsy from the stenotic lesion showed calcified eggs of Schistosoma japonicum with no malignant findings, we suspected malignant involvement, so we performed a descending colectomy with regional lymph node dissection. Pathological examination revealed a moderately differentiated adenocarcinoma. The colon cancer was diagnosed as pT4bN0M0, Stage IIc. The patient’s history as a resident of one of the formerly endemic areas of Japan suggests that he may have carried S. japonicum for a long time, and that it may have contributed to carcinogenesis
